Fuller, lifted breasts without implants: that possibility is drawing more women to autologous augmentation every year. A 2019 study published in Gland Surgery found that patients who underwent mastopexy with autologous tissue scored higher on long-term satisfaction than those who received implants alongside their lift. For women who want a fuller, more youthful breast shape without introducing a foreign device into their body, that finding speaks volumes.

The Projection and Fullness You Choose

Mastopexy with autologous augmentation is a breast lift that uses your own tissue to restore upper pole fullness. Rather than discarding the excess skin and breast tissue removed during a standard lift, Dr. Nodwell strategically repositions and sculpts that tissue into the upper and medial portions of your breast, creating a rounder, perkier contour with improved projection.

The result is a lifted breast that looks and feels entirely natural, because every bit of it is you. If your breasts have lost volume and dropped over the years but you prefer to avoid implants, this technique closes the gap between a traditional breast lift and a breast augmentation and lift without requiring a prosthetic device.

Why Do Ottawa Patients Choose Mastopexy With Autologous Augmentation?

A standard breast lift excels at correcting ptosis, but it typically reduces breast volume slightly as part of the reshaping process. For women who already feel their breasts have lost fullness after pregnancy, nursing or weight changes, that trade-off can be discouraging. You want your breasts higher on your chest, yes, but you also want them to look full and proportionate once they get there.

Implant-based augmentation solves the volume issue, yet many patients have valid reasons for wanting to avoid implants altogether. Concerns about capsular contracture, the possibility of implant replacement surgery years down the road, and a growing preference for natural tissue solutions have all fuelled interest in autologous approaches. Mastopexy with autologous augmentation fills that middle ground. It lifts, reshapes and adds projection using only what your body already provides.

Dr. Nodwell's fellowship training in microsurgery and advanced reconstructive breast techniques at the University of British Columbia provided him with a detailed understanding of tissue vascularity and flap design, which is precisely the skill set that enables this procedure.

What Does a Mastopexy With Autologous Augmentation Treat?

1

Upper Pole Hollowness

A flat or concave appearance across the top of the breast where volume has migrated downward

2

Breast Ptosis

Sagging tissue and nipples that sit at or below the breast crease

3

Nipple Malposition

Nipples that point downward or laterally rather than forward

4

Deflated Contour

Breasts that look empty or pancake-like despite having adequate tissue overall

5

Mild to Moderate Asymmetry

Differences in breast shape or position that affect proportion

What Are My Options for Breast Lift With Autologous Augmentation?

Because this procedure relies on your existing breast tissue, the approach Dr. Nodwell takes depends on how much tissue is available, where it currently sits and the degree of correction your situation requires.

Incision Patterns

The incision pattern follows the same principles as a traditional mastopexy. Dr. Nodwell will recommend either a vertical (lollipop) or inverted-T (anchor) incision based on the amount of skin to be removed and the degree of repositioning required. Your consultation will determine which approach gives you the best outcome with the least visible scarring.

Child Slide

Dermal Flap Auto-Augmentation

Dr. Nodwell creates a pedicled flap from the lower breast tissue that would normally be removed during a lift. This flap is rotated and repositioned into the upper pole, where it is secured to create internal projection and fullness. The technique preserves blood supply to the repositioned tissue, which is essential for long-term viability and shape stability.

Child Slide

Auto-Augmentation With Fat Grafting

When additional volume refinement is desired beyond what tissue rearrangement alone can achieve, Dr. Nodwell may incorporate liposuction-harvested fat grafting. Purified fat cells are injected into targeted areas of the breast to smooth contour irregularities and enhance fullness in specific zones. This combination adds approximately 100 to 150 cc of volume per breast while simultaneously slimming the donor site.

The Nodwell Experience

Breast Reshaping Excellence

What draws me to autologous augmentation is the elegance of the concept. You already have the tissue. My job is to recognise where it belongs and put it there with surgical precision. The reconstructive breast techniques I studied during my fellowship at UBC taught me how to mobilise tissue flaps while protecting their blood supply, and that training translates directly into this cosmetic application.

Every woman's breast anatomy is different, and the volume available for redistribution varies considerably from patient to patient. I assess those variables carefully before recommending this approach so that the final shape meets your expectations. When I do recommend it, the results speak for themselves: a breast that moves, feels and ages naturally because it is made entirely of your own tissue.

The Nodwell Experience

"Surgical excellence is just the beginning."

Am I a Candidate for Mastopexy With Autologous Augmentation?

The ideal candidate has enough native breast tissue to allow meaningful redistribution. Women with moderate ptosis and adequate lower-pole volume tend to see the most dramatic improvement, because there is sufficient tissue available to reshape and reposition into the upper breast.

Your overall health, weight stability and lifestyle factor into candidacy as well. Because pregnancy and nursing can alter results, Dr. Nodwell generally recommends completing your family before scheduling this procedure. Smoking impairs tissue healing and flap viability, so you will need to stop at least six weeks prior to surgery if you are a smoker.

During your consultation, Dr. Nodwell will evaluate your breast volume, skin elasticity, degree of ptosis and body proportions to determine whether autologous augmentation can deliver the result you are looking for. If your desired volume increase exceeds what tissue rearrangement and fat grafting can achieve, he may discuss implant-based options such as an augmentation and lift. Dr. Nodwell will review all of your options to help you select the procedure that fits your goals and lifestyle.

Frequently Asked Questions

How Is Mastopexy With Autologous Augmentation Different From a Standard Breast Lift?

A standard breast lift removes excess skin, repositions the nipple-areola complex and tightens the breast envelope. The process often reduces overall breast volume by a small amount. Mastopexy with autologous augmentation includes all of those steps while adding an additional layer of tissue sculpting that redistributes your own breast glandular and connective tissue into the upper pole. The net effect is a lifted breast that also appears fuller, particularly in the area above the nipple where volume loss is most noticeable.

More

Will I Need Implants in the Future?

The tissue repositioned during this procedure integrates permanently into its new location, so the augmentation component is long-lasting. That said, your breasts will continue to age naturally. If you desire a larger increase in volume than autologous tissue alone can provide, or if future changes prompt you to seek additional enhancement, implants remain an option. Dr. Nodwell will give you an honest assessment during your consultation about the volume increase this technique can realistically achieve for your anatomy.

More

How Much Does Mastopexy With Autologous Augmentation Cost in Ottawa?

The cost depends on the complexity of your surgical plan, whether fat grafting is included and the facility and anaesthesia fees involved. Dr. Nodwell's team will provide a detailed quote during your consultation. We offer financing options through trusted third-party providers to help you manage the investment with monthly payments that suit your budget.

More

Can I Combine Breast Lift and Autologous Augmentation With Other Surgeries?

Yes. Mastopexy with autologous augmentation pairs well with body contouring procedures such as a tummy tuck or liposuction, particularly when fat grafting is part of your breast plan. Combining procedures into a single surgical session means one anaesthesia event and one recovery period. Many women explore this option as part of a Mommy Makeover, which allows Dr. Nodwell to address multiple areas of concern in a coordinated surgical plan.

What Is Recovery From a Breast Lift With Autologous Augmentation Like?

Recovery mirrors that of a standard breast lift, with the addition of mild soreness at any fat grafting donor sites if liposuction was performed. Understanding what each phase of healing involves helps you prepare and plan ahead.

Day of Surgery

After surgery, we will bandage your incisions and place you into a supportive surgical bra. You will return home the same day. Tightness, swelling and mild chest tenderness are all normal during this initial phase. Patients who also had fat grafting can expect some additional soreness at the donor site. Pain medication prescribed by Dr. Nodwell controls discomfort effectively. Have a friend or family member ready to drive you home and remain with you through the first night.

Day of Surgery

Week 1

The first 48 to 72 hours bring the most pronounced swelling and bruising, after which both begin to taper. By day three or four, over-the-counter pain relievers are usually sufficient to keep you comfortable. Gentle walks inside your home help promote healthy blood flow, though you should keep arm movements below shoulder level and avoid picking up anything heavier than a few pounds. Dr. Nodwell schedules your first post-operative visit within this window to review how your tissue is healing.

Week 1

Weeks 2 to 4

Visible bruising clears considerably during this stretch, and you will feel noticeably more like yourself each day. Returning to sedentary work around the two-week mark is typical. Continue wearing your surgical bra day and night except during showers, and follow any scar-care instructions Dr. Nodwell provides once your incisions have sealed. Strenuous movement and upper-body workouts remain off-limits.

Weeks 2 to 4

Weeks 4 to 6

Lower-body cardio and light exercise may resume once Dr. Nodwell gives the go-ahead. Internal swelling continues to diminish, and your breasts gradually adopt a softer, more settled shape. Most patients switch to a wireless sports bra for continued support during this period.

Weeks 4 to 6

After 3 Months

Swelling is largely resolved, and the breast shape you see in the mirror is very close to your long-term result. Scar lines will keep fading for up to a year after surgery, progressing from a pink or reddish tone to a much paler, less visible shade. Dr. Nodwell and his team stay accessible for follow-up visits and any concerns that come up as you settle into your results.

My 32ddds never seemed crazy huge, but the sag caused me rashes that had me scratch so badly I’d bleed. Decided to check with a PS to see what could be done. Found Dr. Nodwell on Google reviews and this site. Liked him immediately.Hadn’t planned on leaving a review, but this site was so helpful to me, figured I’d pay it forward. We did a small reduction, lift with axillary liposuction, and I’m really thrilled with the result.Dr. Nodwell was a pro, made me feel comfortable they the process and did great work. His assistant Emily was gracious with all my questions and quick to respond. This pic is 2 weeks post recovery...obviously, some healing to go and a proper after picture is a couple of months away, but almost no bruising left and the girls are back in their rightful place. I’m looking like I’ll settle in at a 32d. I’m 50, 5’7 and about 140 pounds.First few days were sore from the lipo, and that’s still a bit tender but otherwise, just a bit tired. Dr. Nodwell performed liposcution of my upper and lower abdominals, bra roll, banana roll and inner and outer thighs. My surgery was performed under general anaesthesia at the Carleton Place Hospital. I couldn't be happier with the kindness, knowledge, and sensitivity shown by Dr. Nodwell, all the Carleton Place OR and after care staff and all the support staff and the Ottawa Clinic. Although I am only two weeks into my healing, I am already seeing results. I am confident that I will have the results that years of exercise couldn't achieve. I imagine that I will trust Dr. Nodwell with any of my future aesthetic needs.

I had full body liposuction (waist, inner and outer thighs, tummy, buttocks, back and bra roll) in December 2016. The initial recovery was long, but not overly painful. Dr. Nodwell was upfront about the swelling I would expect and that it would take up to year to see the final results. I am over the two year mark now and I have never felt so confident in my own skin. I had always been physically active but I had gained weight due to poor life choices (too much alcohol) and hormonal changes. Sincer recovery from surgery, I have quit drinking alcohol and I am back to doing the physical activities I love. I have no regrets getting surgery and it has allowed me to make more positive changes in my life in order to keep the results.

I started seeing Dr. Nodwell around 4 years ago, for baby botox. He listened to what I was looking to achieve - a natural refreshed look - and took the time to walk me through what would work best for me, the process, and he made me feel extremely comfortable. I have always been incredibly happy with my botox results! Then, in 2019 we did a second consult for an abdominoplasty! I have two children, was “all belly” and my former 21” waist was stretched beyond natural repair. I am petite and athletic, but the harder I worked out, the worse my abdomen would look which was incredibly frustrating. I had already grown to know and trust Dr. Nodwell through my botox appointments, which really helped me make the decision to finally proceed with my abdominoplasty after 15 years of dreaming about it. I also had him remove a bony growth on my forehead while I was under.Long story short, my only regret is not proceeding with the abdominoplasty sooner, I am absolutely thrilled with my results, and Dr. Nodwell and his staff were a dream through it all.
